Minnesota Residents:
You have two options for scheduling new patient appointments:

1. By telehealth Monday through Friday

2. In person on Mondays at our Minneapolis clinic (4748 Chicago Ave S.)

New patients wishing to be seen in person must select a Monday "IN OFFICE" appointment—we do not currently offer in-person appointments on other days.

Wisconsin Residents:
You have two options for scheduling new patient appointments:

1. By telehealth with Dr. Nanci Miklowski.

2. In person on Mondays at our Minneapolis clinic (4748 Chicago Ave S.) with either Dr. Orchard or Dr. Vilensky.

Note about Follow-Up Appointments for Wisconsin residents:
Currently, patients who establish care in person with Dr. Orchard or Dr. Vilensky should plan to be seen in office at least once per calendar year after your initial visit.

Are you Outside of Minnesota and Wisconsin?

Out-of-state patients can schedule new patient appointments in person on Mondays at our Minneapolis clinic (4748 Chicago Ave S.). Due to Minnesota statutes, we are unable to offer initial telehealth appointments for patients outside these two states.

When booking, please select a Monday "IN OFFICE" appointment, as in-person visits are not available on other weekdays.

Follow-Up Appointments: After your initial visit, you may choose between in-person and telehealth follow-ups. However, at least one in-person appointment per year is required.
