Welcome New Patient!
Naturopathic medicine has so much to offer, and I am excited and honored to have the opportunity to work with you!  As the founder of Be Well Natural Medicine, I look forward to helping you achieve a greater level of physical, mental, and emotional health.
Below you will find links to New Patient forms you must complete before your first visit.  Please bring these forms and any lab results from the last year, if applicable, to your initial appointment.  If you’d like your doctor to have time to review your health info before your appointment, you are welcome to email (doctorliz.nd@gmail.com) or fax (1-866-408-6045) them ahead of time.

Your first visit will take between 75 to 90 minutes. At the end of this appointment, your doctor will send you home with instructions to start you on the road to wellness. Your next appointment will be 4-6 weeks later. You and your doctor will continue to work together until you are at the level of health and wellness you desire. For some of you this may be months, for others, years. At Be Well, we do not manage symptoms. Instead, we are striving to find the roots of your health problems and FIX them. This approach requires more work, and we believe it is worth the effort.

Payment will be due at the time of your visit. Please let us know if you are working within a budget, and we will do our best to prioritize natural medicines for you. If you would like to submit the office visit charges to your insurance company for possible reimbursement, let your doctor know and she will provide you with the necessary information. Insurance may or may not cover these charges and does not typically cover the cost of any natural medicines. If you need to cancel an appointment, kindly give 48 hours notice.
